Please call our office at: (254) 519-8490 to schedule an appointment. New patients will have to come early and fill out a new patient form. They can either fill it out in our office or print the form online in our PATIENT FORMS section and drop it off at our office.
Our office hours are: Monday-Friday 8:30AM to 5:00PM
What To Bring To An Appointment:
Bring your photo ID and your current insurance card(s). Co-pay, co-insurance, and deductibles are due on the date of the service. We accept cash, Visa, and Mastercards. We currently do not accept personal checks.
If you are currently taking any medication, please bring a list of the medications you are currently on.
If you like, you can print out the patient form here and bring it to your appointment.
We accept almost all insurance plans except Medicaid. Please make sure your plans are up-to-date and you bring your insurance card(s) with you when you arrive for your appointment. We will inform you of any co-pays, co-insurances, and unfulfilled deductibles that you have to pay before any medical attention.
Our service fees vary depending on the type and length of each provided services. If you have any questions about the cost please contact our office.
Cancel An Appointment:
In the event that you can not meet your office appointment date, please contact our office at least 1 day prior to avoid a cancellation fee of $25.
